Highlights

  • The Health Fund will reimburse you for up to $50 per massage therapy visit, up to 12 visits a calendar year, for each eligible enrolled family member.
  • There is no provider network for massage therapy services.
  • To receive benefits for massage therapy services, return a completed Massage Therapy Claim Form and attach an itemized statement from your licensed massage therapist.
  • The statement must include your therapist’s name, address, license number and tax identification number (TIN).
