Please note the Fund Office holiday schedule for Christmas week:
*Wednesday, December 24: Closing to members at 11:00 a.m.
*Thursday, December 25: CLOSED
*Friday, December 26: Closing to members at 11:00 a.m.

Regular office hours will resume on Monday, December 29.

The Fund Office is open to members during the hours of 8:00 a.m. to 3:30 p.m. each weekday, excluding holidays.

Highlights

  • The Health Fund will reimburse you for up to $50 per massage therapy visit, up to 12 visits a calendar year, for each eligible enrolled family member.
  • There is no provider network for massage therapy services.
  • To receive benefits for massage therapy services, return a completed Massage Therapy Claim Form and attach an itemized statement from your licensed massage therapist.
  • The statement must include your therapist’s name, address, license number and tax identification number (TIN).
