Holiday Office Hours
Please note that the Fund Office will close to members at 1:00 PM on Thursday, July 2, and will be closed on Friday, July 3, in observance of Independence Day.
We will resume our normal business hours on Monday, July 6.

The Fund Office is open to members during the hours of 8:00 a.m. to 3:30 p.m. each weekday, excluding holidays.

Highlights

  • The Health Fund will reimburse you for up to $50 per massage therapy visit, up to 12 visits a calendar year, for each eligible enrolled family member.
  • There is no provider network for massage therapy services.
  • To receive benefits for massage therapy services, return a completed Massage Therapy Claim Form and attach an itemized statement from your licensed massage therapist.
  • The statement must include your therapist’s name, address, license number and tax identification number (TIN).
